What is Mobile Integrated Healthcare?

Mobile Integrated Healthcare (MIH) is a healthcare program designed to help patients receive additional support and medical care in the comfort of their own homes.

As part of your care team, specially trained Hartford HealthCare EMS/American Ambulance MIH paramedics will visit you at home to provide assessments, education, and select clinical services based on a care plan developed by your hospital physician, specialist, or other healthcare provider.

Our goal is to help you manage your health, support your recovery, and help you stay safely at home with additional support whenever possible.

How Does the Program Work?

1. Your hospital team or physician develops a care plan based on your medical needs.
2. Hartford HealthCare EMS/American Ambulance MIH paramedics visit you at home according to that plan.
3. During visits, paramedics may:

• • Monitor your condition and vital signs
  • Perform health assessments
  • Review your medications and treatment plans
  • Provide patient education and coaching
  • Identify concerns early and communicate with your healthcare team
  • Provide medications and treatments in the home
  • Help connect you with additional healthcare resources when needed

4. Information is shared with your healthcare providers to support coordinated care.

How are Patients Enrolled?

Patients who may benefit from additional support after leaving the hospital are identified by their hospitalist, specialist, or other healthcare provider during the discharge process.  Participation in the MIH program is completely voluntary.

Enrollment Process

• Your physician determines whether the MIH program may be beneficial as part of your recovery or ongoing care.
• Before discharge, you will be asked whether you would like to participate in the program.
• Patients who choose to enroll will provide consent to participate.
• The MIH team will contact you while in the hospital or shortly after your transition home to schedule your first home visit.
• The number and content of visits is determined by your individualized care plan and may range from a single visit to multiple visits over time.
• You may choose to discontinue participation in the program at any time.

Who Will Visit Me?

Hartford HealthCare EMS/American Ambulance MIH paramedics are experienced healthcare professionals specially trained to provide medical assessments, patient education, and supportive care in the home setting.

The MIH paramedics work under physician-directed care plans and serve as an extension of your healthcare team to help support your recovery and ongoing health needs.

Connecting patients, providers, and community resources to deliver care where it matters: at home.

Our Basic Life Support (BLS) Ambulance authorities are issued and inspected by the Connecticut Department of Public Health Office of Emergency Medical Services. Our BLS ambulances are staffed with two Emergency Medical Technicians certified in pre-hospital care and equipped to provide basic emergency care. Their capabilities include, but are not limited to: monitoring vital signs, patient assessment, airway management, oxygen and other misc. medication delivery, application of dressings and bandages, splinting of extremities, spinal immobilization, and administering CPR including the use of an AED.

Our Advanced Life Support (ALS) Ambulance authorities are issued and inspected by the Connecticut Department of Public Health Office of Emergency Medical Services. Our ALS ambulances are staffed with a minimum of one Emergency Medical Technician (EMT) and one Paramedic. 

Paramedics are licensed healthcare professionals in pre-hospital care and their capabilities include, but are not limited to: advanced airway management, cardiac monitoring and defibrillation, IV administration, and administration of select life-saving medications.

This level of service is provided by a team member trained as a Paramedic. An intercept is performed when American is requested to respond along with another EMS organization that operates at the BLS level. An American Paramedic will meet up, “intercept”, with the transporting BLS organization and will get on their ambulance to provide ALS care for the patient until they arrive at the destination hospital.
